Acute graft-versus-host disease (GVHD) occurs after allogeneic hematopoietic stem cell transplant and is a reaction of donor immune cells against host tissues. Activated donor T cells damage host epithelial cells after an inflammatory cascade that begins with the preparative regimen.GVHD usually goes away a year or so after the transplant, when your body starts to make its own white blood cells from the donor cells. But some people have to manage it for many years.Chronic graft-v-host disease (chronic GVHD) is a frequent cause of late morbidity and death after bone marrow transplantation (BMT). The actuarial survival after onset of chronic GVHD in 85 patients was 42% (95%Cl = 29%, 54%) at 10 years.
Acute GVHD disease usually involves a distinct set of clinical symptoms and signs that include dermatitis, or skin inflammation, is characterized by an itchy, red, and possibly painful rash. Some 62% of BMT patients survived at least 365 days, and of those surviving 365 days, 89% survived at least another 365 days. Of the patients who survived 6 years post-BMT, 98.5% survived at least another year. 
Disease recurrence is a devastating event after allogeneic hematopoietic stem cell transplantation as treatment for acute myeloid leukemia (AML). Median time to relapse is approximately 4 months and the majority of relapses occur within 2 years after transplant. The prognosis is usually poor.
In patients with MRD measured after the transplant, the survival rate dropped to 35% leukemia-free and 55% overall. Another study on adult survivors of bone marrow transplant revealed lower patient quality of life when any of the following conditions are present: severe, chronic GVHD. lower performance. it's easier to collect stem cells from the bloodstream than bone marrow. your treatment team can usually collect more cells from the bloodstream. blood counts tend to recover quicker following a stem cell transplant.
